A company is looking for an RN Telephonic Case Manager - Remote in NY.
Key Responsibilities :
Make outbound calls to assess members' current health status
Identify gaps or barriers in treatment plans and provide patient education for self-management
Document findings and coordinate care for members, including post-discharge follow-up
Required Qualifications :
Active, unrestricted RN license
Willingness to obtain multiple state licensure within 60 days of hire
3+ years of RN experience in a hospital or acute care setting, or as a telephonic Case Manager
Computer proficiency, including data entry skills and navigating a Windows environment
Designated quiet work space and access to secure high-speed internet at home
