We are distinct from typical primary care providers and are experiencing rapid growth. We need dedicated individuals like you to join our clinical operations team as an Advanced Practice Nurse. You will play a vital role, providing direct patient care in our medical centers either virtually or in the home setting, based on your assignments.

Under the guidance of the Regional Chief Clinical Officer, you will have opportunities to train new Primary Care Providers and provide essential bridging care to patients. Your responsibilities will include :

• Conducting comprehensive geriatric assessments, documenting medical histories, and performing physical examinations.
• Diagnosing and treating a variety of conditions, providing health education, making physician referrals, and managing patient cases with follow-ups.
• Ensuring thorough documentation that meets our high standards for quality, service, productivity, and teamwork.
• Participating in clinical rounds and conferences, and contributing to comprehensive progress notes and summaries.
• Collaborating with a multidisciplinary team to ensure optimal patient care outcomes.

As a certified nurse practitioner, you will function independently while also consulting with our licensed physicians. You’ll be committed to upholding strict departmental objectives, maintaining regulatory compliance, and delivering quality patient care in accordance with established policies and procedures.
