[job_card.job_description]OR surgical tech proficient in ortho, neuro, general, robotics, urology, and vascular. Primary Source Verification of professional license and / or certification, as required for the position ran within 30 days of start updated upon expiration. Licensures must be active for the State of Louisiana for assignments in Louisiana. FMOLHS Skills Checklist : Unit specific to the contractor’s scope of practice within one year prior to start date; Updated Annually. FMOLHS Competency Exam : Copies of exams needed, within year of start with passing score of 80% or higher. FMOLHS Core Competencies : Must be current within a year prior to start with passing score of 80% or higher. Includes HIPAA, Patient Bill of Rights, Safety Orientation, Blood borne Pathogens, Fire Safety, Infection Control, Elder Abuse and Neglect, Age Specific Care, Cultural Diversity, Advance Directives; renewed annually. Proof of 2 MMR immunizations, positive titer or Declination accepted. Fit Test : Must be one of the 3 mask fit types. N95- 3M 1860, 1860s or 1870+. Must be completed within 1 year prior to start. Updated annually. Proof of 3 Hepatitis B immunizations, positive titer or Declination accepted. Influenza Vaccine required for flu season 10.1.2025 - 4.30.2026 with medical or religious exemptions accepted. TB Test : Proof of negative Blood test or Skin test within one year prior to start or 2-Step TB Skin Test within 30 days and 1 year. Health Statement signed and dated within 1 year of start stating traveler is able to work without restrictions. Proof of 2 Varicella immunizations, positive titer or Declination accepted. Proof of Tdap vaccination within past 10 years or Declination accepted. Annual TB Questionnaire required within 6 months of start. FMOLHS Criminal Background Check Results must be completed within 30 days prior to start including SSN Address Trace, County Criminal Searches, National Criminal check, NSO, OFAC, OIG, and SAM. Negative 10 Panel Drug Screen within 30 days prior to start. FMOLHS Eye Exam & Color Vision : Ishihara Color Test AND Snellen Vision test within 1 year prior to start. Schedule : 0630-1500, Monday-Friday; call required. OR surgical tech proficient in ortho, neuro, general, robotics, urology, and vascular. FMOLHS Skills Checklist FMOLHS Competency Exam FMOLHS Core Competencies Proof of COVID-19 vaccine or exemption accepted.
