• Monday - Friday, 8 am - 5 pm schedule with excellent work-life balance
• See 10 - 12 patients daily with strong APP support
• Minimal call with nurse triage support system
• Value-based care experience preferred
• Board-certified or board-eligible in internal medicine with geriatric focus required
• Experience with HEDIS measures and HCC coding valued
• Spanish language skills appreciated but not required
