Overview
About You In Home Care is seeking a skilled, compassionate, and professional Registered Nurse (RN) to provide clinical support and oversight across multiple partner facilities throughout Louisiana . In this role, you will deliver hands-on nursing care, assess resident health, coordinate care plans, and ensure compliance with facility and state regulations.
This position involves direct clinical duties , including medication administration, monitoring vital signs, assisting with treatments, and supporting caregivers in delivering high-quality, patient-centered care. The ideal candidate is an experienced RN who enjoys working in a facility-based environment , traveling between locations, and ensuring consistent clinical standards across multiple sites.
Key Responsibilities
Clinical Care & Oversight
Provide direct nursing care to residents in accordance with established care plans and scope of practice.
Administer medications, treatments, and therapies as ordered and within RN scope.
Monitor and document resident health status, including vital signs, lab results, and clinical changes.
Recognize and respond to changes in resident condition promptly, escalating concerns to physicians or supervisors as needed.
Collaborate with caregivers and facility staff to ensure residents' clinical and personal needs are met.
Care Coordination & Communication
Develop, review, and update individualized care plans in collaboration with caregivers and facility staff.
Communicate effectively with residents, families, physicians, and the agency team regarding clinical updates or concerns.
Maintain accurate and timely documentation in Electronic Health Records (EHR) and other required forms.
Ensure compliance with Louisiana Department of Health (LDH) regulations, HIPAA standards, and agency policies.
Facility Support & Quality Assurance
Assist in training and guiding caregivers on clinical best practices.
Participate in quality audits, ensuring care meets regulatory and agency standards.
Identify and report safety hazards or concerns to facility management and agency leadership.
Support smooth clinical operations across multiple facilities, adapting to the needs of each location.
Qualifications
Active and valid Louisiana RN license in good standing
Minimum 3 years of RN experience , preferably in long-term care, assisted living, or multiple facility settings
Clinical skills in medication administration, patient assessment, wound care, and basic procedures
Strong communication, problem-solving, and leadership skills
Proficiency with EHR systems and documentation requirements
Valid driver's license and reliable transportation for travel between facilities
Ability to pass background check, drug screening, and maintain CPR / BLS certification
Preferred Experience
Prior experience in long-term care, memory care, assisted living, or similar facility-based nursing
Experience training or mentoring caregivers in clinical practices
Familiarity with Louisiana state facility regulations and reporting requirements
Work Environment & Schedule
Facility-based clinical work across multiple partner sites
Flexible schedules : full-time, part-time, or PRN coverage
Direct patient care, assessments, and medication administration required
