Medical Director - Geriatric Medicine

We are seeking a dedicated full-time Medical Director for our innovative Program of All-Inclusive Care for the Elderly (PACE). This role offers an exciting opportunity to provide comprehensive medical oversight and quality care coordination for seniors with chronic care needs, helping them maintain independence while living safely in their communities. Our facility champions the belief that seniors should be served in the community whenever possible, ensuring dignity and respect in every interaction.
Job Responsibilities
- Conduct history and physicals on new participants and perform event-based and periodic reassessments.
- Develop medical treatment plans and provide required services across various participant locations.
- Interview and evaluate applicants and their caregivers for the PACE Organization.
- Discuss medical conditions and prognosis with participants and caregivers, guiding future care planning.
- Collaborate in establishing nursing treatment programs.
- Contribute as a member of the leadership team to achieve operating goals.
- Engage with the interdisciplinary team, offering insights on diagnoses, treatments, and cognitive capabilities.
- Educate participants and families about health maintenance and chronic conditions.
- Supervise the interdisciplinary team’s health care programs.
- Conduct physical exams, order tests, and record evaluations systematically.
- Participate in provider on-call rotation as needed.
- Note : Not accepting Locums or Visa Candidates. Prior experience is REQUIRED.
Qualifications
Current and active plenary physician license to practice in Massachusetts - [REQUIRED]Board Certification in Internal Medicine or Family Practice - [REQUIRED]Board Certification in Geriatrics - [PREFERRED]Experience or additional qualifications in geriatrics - [PREFERRED]CDS Certification and DEA Registration - [REQUIRED]Minimum of one (1) year of experience working with a frail or elderly population - [REQUIRED]Effective skills in physical assessment and chronic disease management for older adults - [REQUIRED]Desire to work in a creative alternate health care setting - [REQUIRED]Able to manage changing priorities according to participant needs - [REQUIRED]License and Certification Requirements
