We are looking for a dedicated and experienced Hospice Clinical Liaison to enhance our patient care team. In this pivotal role, you will facilitate efficient intake and referral processes that prioritize the needs of patients, families, and healthcare professionals. Your contributions will be crucial in fostering strong relationships, managing admissions, and promoting awareness of our hospice services within the community.
Key Responsibilities:
- Receive referrals from a variety of sources such as hospitals, health providers, and case managers.
- Evaluate patient eligibility for hospice care, analyzing medical records and conducting clinical assessments.
- Educate patients and families about hospice services, providing comfort and clarity.
- Handle all necessary documentation while coordinating the intake and admission process to meet all requirements.
- Act as a liaison between referral sources and the hospice care team for smooth transitions and effective communication.
- Conduct regular visits to hospitals and facilities, ensuring strong visibility within the healthcare community.
- Build enduring relationships with referral partners and seek out new opportunities to enhance hospice service awareness.
Qualifications:
- Education: Licensed Nurse with 3-5 years of hospice experience required; BSN preferred.
- Experience: Minimum of 3 years in hospice settings; prior experience in discharge planning or case management is advantageous.
- Certifications: Current RN license in the state of employment and CPR certification are required.
- Skills: In-depth knowledge of hospice care, superior assessment and documentation skills, and strong interpersonal communication abilities are essential.
