ASSIGNMENT SUMMARY
The Document Clerk is responsible for the accurate and timely processing and retrieval of the patient's health records, will support Medical Records Requests, Fax Management and other work pertaining to the Patient Care Coordination Team.
ESSENTIAL FUNCTIONS / RESPONSIBILITIES
- Retrieves, collects and prioritizes all request for medical records made through the organization, patients and affiliates
- Manages incoming medical records requests received via fax, email and / or mail & issues medical files to persons and agencies according to laws and regulations in a timely manner
- Verifies legitimacy, accuracy and authority of all such requests and then delivers the information following established policies and procedures. This includes responding to requests from both internal and external sources, retrieving the records, and transmitting them as needed
- Requests medical records from third party vendors, as needed
- Safeguards patient records and ensures compliance with HIPAA standards
- Scans and files all consult and hospital reports in appropriate sections within the EMR system following departmental procedure
- Maintains filing system of medical charts
- Proposes improvements for maintaining medical records procedures
- Ensures seamless troubleshooting of issues related to the maintenance and timely updates of medical records
- Contributes to departmental quality initiatives or similar projects
- Assists with departmental audits and investigations
- Serves as back-up for referral coordinators when needed
- Performs clerical duties including answering phones, responding to emails and actions
- Exemplifies caring and compassionate customer service to create an excellent patient experience
- Records and tracks all records requests, releases and authorizations within the Electronic Medical Records system & sends monthly updates to the Specialty Care Manager
- Review, develop and implement standards and procedures for processing medical records requests with administrative support
- Processes and complete all subpoena requests in a timely manner
- Performs all other duties as assigned
MINIMUM QUALIFICATIONS
