Description : Position Summary :
- Provides in-home, community-based and / or telehealth treatment services to a caseload of individuals aged 55 and over who have a mental health diagnosis.
Essential Duties and Responsibilities :
Responsible for providing clinically appropriate psychoeducation, advocacy and coordination of services to assigned clients.Engages clients in the development of annual treatment goals, clinical reviews and completion of all other documentation related to the client’s treatment.Maintains communication with clients, caregivers, and helping professionals / agencies serving the client through phone calls, written communications, and in-person meetings.Has a working knowledge of entitlements / benefits, housing and other community resources needed to support clients.Provides necessary direct service hours to meet the performance standards set by the Behavioral Health Services Department.Completes and submits all documentation and client data in a timely manner according to department / agency protocols and funder requirements.For licensed staff :
Performs assessments / reassessments as assigned.Provides support to Day Treatment Group as facilitator on an as-needed basis.Other Functions and Responsibilities :
Maintains documentation of client records according to agency, regulatory and funding requirements, including but not limited to CARF, Ohio Dept. of Behavioral Health, Ohio Dept. of Medicaid, and the Cuyahoga County ADAMHS Board.Provides information and referrals to other agency home and community-based service (HCBS) programs as appropriate.Works to maintain good working relationships with co-workers, management as well as other agency program and service staff.Attends agency meetings as required.Requirements :
Bachelor’s degree in Social Work or related field.Current and unrestricted Ohio licensure as an LSW is preferred.Three years’ experience in social service field is preferred.Demonstrated knowledge of mental illness [as described by the Diagnostic and Statistical Manual V of the American Psychiatric Association].Must have a current valid Ohio driver’s license and automobile insurance and access to personal transportation.Must have excellent verbal, written and electronic communication skills.Demonstrated proficiency in use of computer programs (Microsoft Outlook and Office).Demonstrated proficiency in use of electronic health records.Strong planning and organization skills.Understanding of complex housing needs.Attention to detail and accuracy are essential, as are abilities to prioritize work and consistently meet deadlines.
