Generations Home Healthcare, a premier home healthcare agency located in Gladstone, NJ, is seeking a compassionate and professional Registered Nurse (RN) to join our growing private duty nursing team. This position covers Somerset, Essex, Union, Morris, Bergen and Hunterdon Counties depending on which locations work best for you. Key Responsibilities :

• Conduct initial client assessments to determine care needs
• Develop and implement individualized client care plans
• Case management, including building longlasting relationships with assigned clients
• Perform skilled nursing duties such as wound care and medication management
• Perform re-assessments and care plan updates to maintain the client's optimal health
• Orient caregivers at the start of a case as applicable
• Provide in-person and on-call support to clients as needed
• Maintain client records and documentation
• Educate clients and families about healthcare management options
• Maintain accurate documentation of nursing care provided
• Conduct caregiver competency assessments as needed
• Supervise and visit field staff as necessary

Requirements :

Valid New Jersey Registered Nurse (RN) license

CPR / Basic Life Support certification

Proof of eligibility to work in the U.S

Proficient computer literacy

Valid driver's license and reliable car

Ability to work independently and as part of a team

Strong clinical and critical thinking skills

Exceptional communication and interpersonal skills

Ability to lift up to 50 lbs

One year of nursing experience preferred

Home care experience preferred
