Job Summary

This Program Coordinator position has primary responsibility of supporting the development and maintenance of the Transitions of Care program (ToC). This position reports to the Community Health Program Director. This position will assist in managing relationships and engagement strategies with the City of Seattle, Home and Community Services, Hospital Providers, and various existing partnerships in the community. ToC services include accepting referrals for individuals who have recently discharged or will be discharging from the hospital and assist with support of Care Transitions, coordination and monitoring of formal and informal service delivery, linkages to other Neighborhood House and community-based services, and ongoing case management support. The Program Coordinator plays a critical leadership role and shares responsibility for the implementation and operations of important Community Health Services.

The Program Coordinator will play an essential role in developing and maintaining procedures, tracking systems, and quality assurance measures that ensure smooth program operations. They will also be responsible for accurate data entry and reporting within the State system Getcare, as well as monitoring service outcomes and client satisfaction.

The ideal candidate will have a strong background in care coordination and case management, with experience maintaining program compliance and working across multiple data systems. The coordinator must be highly organized, detail-oriented, and capable of working independently while collaborating effectively with partners across healthcare and community networks.

Responsibilities

Systems Support (50%)

  • Assist with the design, development, and ongoing execution of the program's systems to ensure that the program is highly effective and complies with all relevant policies.
  • Assist with the development and updating of key project documents to support the program, including policies and procedures, evaluations, satisfaction surveys, etc.
  • Liaise with program funders and audit staff as necessary to maintain program compliance.
  • Assist and prepare for regular monitoring and complete audit tasks as needed.

Ongoing Monitoring (50%)

  • Assist with data review and analysis; ensure data is complete and correct; complete additional data entry as required.
  • Maintain up-to-date performance tracking tools.
  • Identify gaps or program performance issues in a timely manner and problem solve with management and team members to quickly address issues.
  • Develop and deliver team trainings and complete one-on-one coaching with staff to ensure reporting compliance; work closely with program leadership to adjust training based on individual staff needs to maximize efficiency and effectiveness.
  • Prepare, review, and submit program reports as assigned.
  • Perform other duties as assigned. This may include resource referral and light case management as needed.
  • Qualifications

    Minimum Qualifications

  • Degree in social work or a related field; OR four years of experience in social services programming.
  • Familiarity with non-profit compliance systems and programmatic monitoring and evaluation.
  • Good listening and oral / written communications skills.
  • Demonstrated ability to thrive in a culturally diverse workplace dedicated to equity and inclusion.

  • Ability to analyze data and develop tables and charts to make data accessible to all.
  • Knowledge of and experience providing training in a variety of settings and utilizing adult learning techniques.
  • Strong skills in navigating client management systems and the Microsoft Suite of programs including Excel.
  • Highly detail oriented and demonstrated ability to handle confidential data / information appropriately.
  • Highly positive and enthusiastic style capable of motivating others.
  • Ability to work as a team member in a multicultural organization and community.
  • Must have reliable, independent transportation for frequent travel to client homes and between sites.
  • This position is expected to work at a Neighborhood House office in Tukwila and / or White Center for 2-3 days each week.
  • Required Credentials

  • Must pass Neighborhood House background check requirements.
  • Position requires Washington State bi-annual criminal background re-check.
  • Post-Hire on-the-job Training Requirements

  • Current First Aid Certificate within 30 days of hire.
  • Current CPR Certificate within 30 days of hire.
  • Sexual Harassment Training within 30 days of hire.
  • Child Abuse and Neglect training (within 2 weeks of hire).
  • Mental Health First Aide training within 60 days of hire.
