Responsibilities

Provide direct nursing care to residents under the supervision of an RN or physician

Administer medications and treatments accurately and safely

Perform wound care, tube feeding, and trach care as applicable

Document all care and observations using EMR systems

Monitor and record vital signs, changes in condition, and report promptly

Maintain infection control procedures and ensure a safe environment

Collaborate with interdisciplinary teams to support individualized care plans

Assist residents with daily living activities as needed

Requirements

Active Oregon LPN license in good standing

Minimum 6 months – 1 year experience in Skilled Nursing, Post-Acute, or Long-Term Care

Current BLS / CPR certification

Ability to work independently and in a team

Strong communication and clinical documentation skills

Reliability and professionalism are a must
