Post Acute Care Physician / Primary Care Physician - Augusta, ME

Vantage Healthcare is seeking Board Certified Geriatricians, Internists, and Family Practitioners to join our group of dedicated, experienced practitioners who provide comprehensive on-site medical care to patients in post-acute, rehabilitation, long term care, continuing care communities and assisted living centers.

We provide care to patients across the spectrum of post-acute facilities from short term rehabilitation patients to complex geriatric patients in skilled nursing facilities. Our goal is to remove barriers, improve access and provide state of the art care to achieve the best clinical outcomes for our patients and the facilities we serve.

As a member of the Vantage Healthcare team, you will be part of an exciting concept in the way post acute care is delivered. Through Vantage Healthcare's structured model of care, we improve our facility partner's cultural, clinical and financial performance.

• Essential duties and responsibilities of a Continuity Physician include :
• Provide face to face primary care services
• Participate in monthly educational lecture series and clinical outcomes reviews
• Collaboration with Nurse Practitioner / Physicians Assistants
