Matrix Providers is hiring a Clinical Pharmacist to join our team of talented professionals who provide health care services to our Military Service Members and their families in at Travis Air Force Base, California.

  • Employment Status: Full-Time, Short term-Coverage position (minimum of 12 weeks)
  • Compensation: This is an hourly position, paid bi-weekly.
  • Schedule: Services are provided between 0800-1700, Monday – Friday. Shifts will normally be scheduled for a 9-hour period, to include an uncompensated 60-minute lunch break.

The Clinical Pharmacist must have and meet the following:

  • Degree/Education: Degree: Doctor of Pharmacy ( Graduate from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE). Internship/Residency: Completion of a clinical pharmacy residency or fellowship accredited by the American Society of Health System Pharmacists or American College of Clinical Pharmacy or documentation of appropriate education, training, and/or CME in the practice of clinical pharmacy.   • Experience: A minimum of three years of experience within the last five years working as an infectious disease clinical pharmacist is required or completion of a post-graduate year two specialty pharmacy practice residency in Infectious Diseases. Expert knowledge of evaluative, analytical, investigative, qualitative and quantitative techniques to conduct review and analysis of pharmacoeconomic issues such as cost effectiveness, cost minimization, quality life-years, overall drug therapy costs, budget forecasting, and review of automated ordering in order to evaluate performance pricing and arrive at valid recommendations and policies. Exercises skill in planning, organizing and directing outcomes of research studies and economic analyses within the pharmacy and advises the management of budget and program implications. Knowledge of complex pharmacological protocols to include use, method of administration, dosing, schedule, adverse reactions, and complications for acute episodic and chronic illnesses. Mastery to apply experimental theories and new developments to the resolution of complex health care not susceptible to treatment by accepted methods; or to make decisions or recommendations significantly changing or developing protocols. Knowledge of the full range of professional medical principles and practices and their application to provide a broad range of direct primary health care services (, recognize signs and symptoms of illness, interpret lab results, and assess patients' medical conditions and health maintenance status.) Knowledge and ability to apply emergency and resuscitative measures. Independently make decisions and implement proper emergency measures after identifying and interpreting life-threatening conditions. Initiates life-saving measures, which include, but are not limited to, basic cardiopulmonary resuscitation, calculating dosages and mixing potent and potentially lethal drugs and administering these drugs. Thorough knowledge to provide preventive anticipatory care to adults in nutrition, care of common illnesses, growth and development. Knowledge of principles of history taking, physical and psychosocial, using the problem-oriented approach. Knowledge of a variety of common, acute and chronic illnesses, their causes, prevention, treatment and other conditions which may affect the course of the illness. Knowledge of medical care and treatment sufficient to perform complete physical examinations and administer a variety of medical treatments. Knowledge of and the ability to apply principles of infection control and safety. Ability to communicate effectively in writing including, but not limited to, patient records, standing operating procedures, staff records, reports, minutes, electronic means, and databases. Utilizes various automated systems to include Composite Healthcare System (CHCS), Ambulatory Data Module (ADM), Outlook, Computerized Information system (CIS), and Preventive Healthcare Application (PHCA). High level of interpersonal skills to be able to work closely with patients and the MTF staff. Possess skill in the utilization of good human relationship principals and techniques to resolve issues and ensure successful implementation of proposals. Must be skilled in briefings and preparation of study reports and briefing charts. Proposals involve substantial agency resources, require extensive changes in established procedures, or may be in conflict with the desires of other activities.
  • Licensure/Registration: Current, full, active, and unrestricted license to practice as a pharmacist.
  • Certification: Basic Life Support for Healthcare Providers, American Heart Association, or American Red Cross Healthcare Provider Course.

Clinical Pharmacist Core Job Duties:

  • Performs duties as a Clinical Pharmacist in direct support of the provision of healthcare to patients admitted to the inpatient wards and intensive care units of the hospital in addition to outpatient clinics.
  • The Antimicrobial Stewardship/Infectious Diseases Clinical Pharmacist works as a part of a multidisciplinary team and co-leads the Antimicrobial Stewardship Program (ASP) with the ASP Medical Director. Monitors and manages in the development, implementation, and monitoring of the Antimicrobial Stewardship Program (ASP). This includes but is not limited to antibiotic guidelines, clinical pathways, policy development, formulary management, restriction criteria, educational efforts, antimicrobial usage, and resistance monitoring. The Antimicrobial Stewardship/Infectious Diseases Clinical Pharmacist will be part of a multidisciplinary team that includes physicians, nurses, microbiology, infection prevention, and information technologists. The Antimicrobial Stewardship/Infectious Diseases Clinical Pharmacist will co-lead the Antimicrobial Stewardship Program with the ASP Medical Director to coordinate all efforts and activities of the stewardship team with the goal of ensuring that facility metrics and goals are accomplished. The Antimicrobial Stewardship/Infectious Diseases Clinical Pharmacist will serve as a medication therapy expert and advises physicians, medical residents, nurses, and patients on antimicrobial use to optimize patient outcomes and ensure these agents are used in a safe, cost-effective way. Develops core strategies for Antimicrobial Stewardship Program to include Key Performance Indictors related to Antimicrobial Utilization and Stewardship. Review and develop best practice recommendations for all areas of antimicrobial management. Clinical pharmacist maintains effective communication with leadership teams on ASP and assists in preparation and implementation of pharmacy staff competencies related to Antimicrobial Management. Plans and provides appropriate treatments, initiating and modifying selective therapies. Writes prescriptions for legend and over the counter medications IAW pharmacy list as approved by the Pharmacy and Therapeutic Committee. Performs treatments authorized by protocol or physician consultant. Periodic evaluation of the proper utilization and dispensing of medications will be accomplished by physician preceptor. Refers patients to supervisory physicians, other hospital departments, support services, or to community social welfare agency and services as appropriate.
  • The role of the Infectious Disease Clinical Pharmacist is to support the facility in a variety of pharmacy benefit management functions that provide integrated support to the direct care and purchased care components within the DoD pharmacy benefit. Incumbent serves as the expert Subject Matter Consultant (SMC) for drug therapy and pharmacy service issues. Accomplishes special quality assurance studies. Performs pharmacoeconomic analyses. Provides advice, professional consultation/opinions and written technical studies and professional reviews in support of the various boards and committees within DGMC involved with evaluation of patient care and treatment. Attends Department or Hospital board or committee meetings. Leads and participates in the ward and pharmacy Utilization Management and Quality Assurance processes. Develops and ensures compliance with defined protocols. Attends Continuing Medical Education (CME) courses and maintains specialty specific CME sufficient to satisfy the requirements of applicable licensing agencies and the Joint Commission on the Accreditation of Hospital Organizations.
  • Conducts ward orientation for new medical, nursing, allied health, and pharmacy students, interns, and residents as they provide care diagnosis and treatment to patients. Also teaches in formal lecture settings, mentors and conducts final evaluation feedback. Prepares, conducts, or oversees the training of physicians, pharmacists, and other health care providers on the use of antimicrobials. Monitors training of residents, interns, and fellows to ensure drug related issues are fully and correctly dealt with in their curriculum and that the required levels of skill have been attained. Educates providers on clinical protocol justification, execution, and implementation. Recommends and initiates changes and updates to instructions, regulations, and other guidance as needed.
  • Attends and provides supporting documentation to assigned committees, projects, and programs representing the pharmacy/medical specialty.
  • Participates in the David Grant Medical Center Continuing Medical Education program. Qualifies annually in the Basic Support Healthcare Provider Course. Develops and conducts health promotional services. Serves in a teaching/coaching capacity in the development of other health care workers. Attends designated department and committee meetings; participates in performance improvement and the implementation of practice guidelines. Requires knowledge of research process and research utilization; promotes scientific inquiry into clinical practice outcomes. Assists and guides others in application of and interpretation of research findings in clinical practice. Will collaborate with designing and conducting research and communicating results through publication. Provides guidance, orientation, and training to the MTF staff.
  • Serves as an expert on pharmaceuticals, pharmacokinetics and pharmacodynamics. Uses extensive and intensive knowledge and experience with drugs and other pharmaceuticals available, their nature, action, and interaction, provides authoritative instructions, advice, and assistance in prescribing and dispensing officials in the MTF on effectiveness, suitability, hazards, and other aspects of their use. As subject matter expert, provides input on streamlining or de-escalation of drug therapy, dose optimization, IV to PO conversion, appropriate utilization protocols, and drug restrictions. Creates antibiograms to improve antibiotic utilization and selection.
  • Responds to requests from the DGMC medical staff, outlying pharmacists and medical staff personnel from MTFs throughout the assigned region to participate in, conduct, or review studies involving complex pharmacotherapeutics regimens. Designs and conducts studies intended to evaluate particular drugs, their effectiveness and suitability, or to study the reaction of diseases, patients, and conditions to drug based treatment. Reviews and recommends pharmacy aspects of all protocols and investigative studies. Closely monitors pharmacy aspects of all experimental and investigative drug use and control. Supports research operations by reviewing protocols for randomization, scientific method, and protection of human subjects.
  • Develops, with physicians, pharmacotherapeutics regimens for especially difficult or complex cases. Independently manages the care of patients within protocols and reference manuals mutually agreed upon with the physician consultant. The clinical pharmacist must be able to competently manage problems in an inpatient ward or intensive care unit setting and is subject to institution wide medical review of practice patterns. Examines and conducts thorough evaluation of patients utilizing laboratory and clinical findings to appropriately treat or refer patients to the appropriate specialty ward. Provides non-urgent care for patients of all ages. Obtains a health history from the patient and/or family/guardian and records findings. Requests special screening tests and laboratory tests and uses the results in assessing the patient’s medical problems. Identifies and manages specific illnesses. Develops, implements, and evaluates the treatment plans. Makes judgments about the use of medications and treatment, prescribing appropriate medications and treatments based on protocols. The infectious disease clinical pharmacist serves as a consultant on cases involving multiple, serious, or life--threatening conditions with conflicting drug requirements, serious drug Reactions or interactions, or other factors not receptive to standard or more common specialized approaches. Monitors progress and response to the regimen, modifying dosages or methods as necessary.
  • Co-lead and manage the Antimicrobial Stewardship Program (ASP) with the ASP Medical Director.
  • Develops, with physicians, pharmacotherapeutics regimens for especially difficult or complex infectious disease cases.
  • Assess patient's response to drug therapy and planning drug therapy based on physician-established diagnoses.
  • Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.
  • Initiate, modify, or discontinue medications for ongoing therapy in cooperation with the medical staff. Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (, aminoglycosides).
  • Administer prescription or non-prescription drugs according to established agreements or MTF protocols.
  • Evaluate medical and medication histories for drug-related problems and adjusting drug therapy accordingly.
  • Consult with other healthcare providers (, physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.
  • Provide medication and healthcare related in service training to clinical staff on new medications on the market; appropriate use of medications; comparison of current medication therapies; and other topics as deemed appropriate by the department head.
  • Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting IAW The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards as appropriate.
  • Prepare and presents medical staff drug utilization reviews (DURs) as deemed necessary.
  • Precept pharmacy residents and students as needed.
  • Review and develop best practice recommendations for all areas of antimicrobial management.
  • Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age specific dosage, and other pertinent data are appropriate for individual patients.
  • Ensure accuracy of all pharmaceutical products prepared or processed by technical staff.
  • Facilitate medical staff drug utilization reviews (DURs) as deemed necessary.
