[job_card.job_description]Perform comprehensive physical therapy evaluations per physician orders Review patient medical history, diagnoses, precautions, and contraindications Develop individualized treatment plans with measurable functional goals Deliver skilled physical therapy treatments in the home setting Reassess patient progress and modify plans of care as needed Communicate patient status and progress with physicians, care teams, patients, and families Recommend and assist with adaptive and durable medical equipment Maintain accurate and timely evaluation, treatment, and billing documentation Ensure compliance with HIPAA, company policies, and state / federal regulations Supervise Physical Therapist Assistants and Therapy Aides per state practice act

Physical Therapist (Home Health) Pay Rate : $57.30 per hour
Physical Therapist (Home Health) Schedule : Monday–Friday | 8 : 00 AM – 5 : 00 PM
- On-Call : 1 Saturday per month
Job Type : Contract
Work Setting : Onsite | Home Health
Mileage Reimbursement : $0.48 per mile
Physical Therapist (Home Health) Job Summary
We are seeking a licensed Physical Therapist with home health experience to provide in-home physical therapy services across Nicholas and Fayette Counties, West Virginia . This role focuses on evaluating, treating, and educating patients to improve functional mobility, safety, and independence in the home environment.
