Job Requirements

  • Education Requirements : Master's Degree in Physical Therapy from an accredited program
  • Certifications Requirements : AHA BLS certification, Wisconsin licensure or pending licensure application, NPI registration
  • Experience Requirements : Minimum of 1 year of home health experience working with adult and geriatric populations
  • Other Requirements : Valid Driver's License with proof of car insurance, strong documentation skills, resilient and adaptable attitude, proficient in EMR systems, and able to manage change effectively

  • Evaluate and develop personalized treatment plans for adult and geriatric patients in a home health setting.
  • Provide direct physical therapy services, including assessments, interventions, and goal setting, to improve patient mobility and functional independence.
  • Maintain accurate and timely documentation of patient visits using NetSmart EMR, ensuring compliance with regulatory and reimbursement standards.
  • Collaborate with interdisciplinary team members and caregivers to coordinate patient care and ensure optimal outcomes.
  • Manage caseload efficiently, with an average of 4.5 visits per day, while adapting to changing patient needs and environment.
