Your Role on the Team

Position Summary :

Under the auspices of UCI Health's Care Management team, the Community Connector supports patients and families by facilitating access to services and assisting with care coordination logistics. This role focuses on practical support tasks such as appointment scheduling, follow-up coordination, and connecting individuals to internal and external resources. Working in close collaboration with the Resource Coordinator, and the Clinical Social Work teams the Community Connector helps create a seamless support experience for patients navigating complex systems of care.

Required Qualifications :

• Must possess the skill, knowledge and ability essential to the successful performance of assigned duties
• Must demonstrate customer service skills appropriate to the job
• Knowledge of Diabetes or Heart Failure disease
• Interest in the community served by UC Irvine Health (A Passion for Caring)
• Effective communication skills
• Ability to read and communicate effectively in English
• Ability to maintain a work pace appropriate to the workload
• Ability to establish and maintain effective working relationships across the Health System
