A company is looking for a Social Worker - LICSW.

Key Responsibilities

Support the behavioral health needs of patients on an assigned caseload using evidence-based models of care

Collaborate with prescribing providers for psychotropic medication management and monitor treatment adherence

Document patient encounters and assessments in electronic health records in a timely manner

Required Qualifications

Master's degree in Social Work

Current Rhode Island LICSW required

2+ years of prior healthcare / behavioral health experience required

Geriatric care experience highly desired

Proficient in Microsoft Office Suite and EMR experience
