Key Responsibilities

Deliver skilled nursing care to home health patients with acute care needs

Complete accurate and timely OASIS documentation

Manage a high-volume caseload of approximately 5–6 patient visits per day

Collaborate with interdisciplinary care teams to ensure quality outcomes

Travel within assigned territory to provide in-home patient care

Required Qualifications

Active RN License (Pending licenses not accepted)

Minimum 2 years of Home Health RN experience

Recent OASIS experience REQUIRED

High-volume caseload experience REQUIRED

Previous Home Health Travel Experience REQUIRED(General travel experience not accepted)

Ability to pass an OASIS competency exam

Preferred Qualifications

Experience with MatrixCare EMR

Strong time management and organizational skills

Familiarity with LA County service areas
