Salary Range : $200,000.00 to $300,000.00 per year

By adhering to Connecticut State Law, pay ranges are posted. The pay rate will vary based on various factors including but not limited to experience, skills, knowledge of position and comparison to others who are already in this role within the company.

Physicians Alliance of Connecticut (PACT, LLC) is a multi-specialty physician group that is in partnership with Hartford Healthcare is seeking two Physicians to work with our PACT Primary Care West Haven offices. The office will be located in Milford.

Summary

Under indirect supervision, provides and manages health care for a specified patient population.

Essential Functions

• Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
• Engages patients and their families or caregivers in understanding, setting and monitoring self management care plans, and documents individualized care plans and care coordination in the patient chart.
• Analyze patients medical history, medication allergies, physical condition and examination to determine best care
• Diagnose medical condition
• Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
• Refers patients to specialists or to internal medicine as appropriate.
• Directs and coordinates the patient care activities of nursing and support staff as required.
• Respect and promote patient rights and privacy.
• Responding appropriately to emergency situations.
• Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
• Participate in specified health promotion, education and / or prevention programs.
• Participates in population health activities as directed by the practice.
• Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and / or infection control standards
