Occupational Therapist (OT) Location: Woodbury, New Jersey
Contract Term: March 1, 2026 - February 28, 2027 (1 Year)
Work Arrangement: Onsite - Regular Resident Care Sessions & Required Meetings

Occupational Therapist (OT) to provide rehabilitative therapy. The OT will evaluate residents' functional abilities, develop adaptive strategies, and restore independence in activities of daily living (ADLs) while ensuring compliance with state and federal care standards.
Key Responsibilities: Clinical & Functional Care - Conduct resident functional evaluations upon admission, quarterly, and when condition changes.
- Develop and implement individualized occupational therapy plans to restore ADL performance.
- Recommend and fit adaptive equipment (wheelchairs, walkers, splints, braces, etc.) and ensure proper documentation.
- Label and track all resident DME; maintain a centralized inventory record.
- Collaborate with the MDS Coordinator for ARD selection and reimbursement optimization.
- Support the Nursing Restorative Program, including monthly reviews with the Restorative Nurse.
- Attend fall meetings, care planning sessions, and quarterly rehab team meetings.
Documentation & Compliance - Utilize the Sigma Care System for all evaluations and progress documentation.
- Update resident medical records and maintain ongoing care notes.
- Notify facility leadership of any service or therapist change affecting residents.
- Ensure all services adhere to NJ and federal occupational therapy standards.
Education & Qualifications: - Licensed Occupational Therapist in the State of New Jersey (Required)
- Active Malpractice Insurance (Required)
- Experience in long-term care or skilled nursing environments
- Familiarity with Sigma Care or equivalent EMR systems
- Knowledge of state and federal rehab documentation regulations
