Job Description

American Traveler is seeking an experienced Physical Therapist for a home health position requiring at least one year of recent home health experience and a valid PT license.

Responsibilities

  • Position is based in home health settings serving patients throughout Pinellas County
  • Patient population includes med / surg, cardiac, orthopedic (hips, knees, shoulders), post-surgical, neurology (including CVA, Parkinson's), respiratory, trauma, ADL needs, and more
  • Caseload of 6-8 skilled home care visits per day with a minimum of 6 points and manager discretion to increase
  • Treats Medicare, Medicare Advantage, and commercial insurance patients
  • Day shift schedule, 5x8 hour shifts from 8 : 00 to 17 : 00
  • Requires 1-2 weekends per contract
  • Use of BayCare Forms, BayCare Scheduling, Ace Timesheet documentation system, and Cerner charting (preferred experience)
  • Laptop, MiFi, and portable printer provided for home visits
  • Self-scheduling of hours and visits based on the needs of the HomeCare office
  • Duties include comprehensive PT assessments, developing home exercise programs, providing patient and family education, monitoring for signs of abuse, and emphasizing safety and injury prevention
  • Responsible for equipment use and recommendations including gait belts, walkers, lifts, collars, wheelchair assessment, supplemental oxygen, and prosthetics
  • Home visits are self-scheduled based on client needs and office requirements
  • Professional support includes access to technology needed for documentation and home care visits
  • Solid color scrubs are required
  • 100-mile radius rule applies; candidates must live at least 100 miles from the facility for eligibility
  • Facility is smoke-free
  • Return travelers must wait at least one year after permanent employment to return
  • Training modules must be completed pre-start and annually, recorded by the facility
  • No RTO requests accepted in advance; time off must be worked out onsite with unit manager
  • Candidates may be required to float within the hospital or to affiliate facilities

    • Requirements

  • Active FL PT license or compact license required at time of consideration; license required if applicable
  • Minimum of 1 year recent home health experience
  • BLS certification required
  • Valid driver's license and personal car required for home visits
  • Strong assessment skills and experience with cardiopulmonary, orthopedic, neurological, and post-surgical cases
  • Proficiency with home exercise programs, equipment safety, documentation, and billing procedures
  • Experience with OASIS documentation preferred
  • Experience with Cerner charting preferred
  • Must be COVID vaccinated or have an exemption form on file
  • Must have flu shot or declination and comply with mask policy
  • Pass / Fail EKG exam (up to 3 attempts) required before start
