Job Summary :

As a Nurse Practitioner or Physician Assistant, you will work collaboratively with a physician to enhance individual primary healthcare services through the diagnosis and treatment of various health conditions.

Responsibilities :

• Collect patient histories and conduct assessments through observation, interviews, and examinations.
• Order, perform, and interpret diagnostic studies.
• Deliver preventative health assessments, screenings, immunizations, and patient care.
• Provide direct treatment and manage health conditions with prescriptive authority or by referring patients to other healthcare providers or community resources.
• Counsel and educate patients and their families on preventative health, treatment options, and available community resources.
• Maintain accurate documentation of healthcare services and nursing processes in line with specialty standards and network policies.
• Ensure effective communication and coordination among case team members, patients, and families.
• Adhere to medical records documentation standards.
• Perform emergency procedures within your training scope.
• Actively participate in departmental initiatives.
• Deliver compassionate care in accordance with Ethical / Religious directives.
• Assist with additional duties as assigned.

Qualifications :

Must possess a Licensed Advanced Practice Nurse Practitioner credential from the Connecticut Department of Public Health before your hire date or job transfer date.

A Master of Science in Nursing is required.

A minimum of 2 years of experience is preferred.
