Utah Licensed Clinical Pharmacist

A company is looking for a Clinical Pharmacist (Temp-to-Hire).

Key Responsibilities

Provide expert medication therapy consultations and optimize patient outcomes

Review and evaluate patient medication profiles and counseling sessions

Assist in orienting new team members and facilitate prior authorizations

Required Qualifications

Doctor of Pharmacy (PharmD) from an accredited college / university

Licensed Pharmacist in good standing, with multiple state licenses preferred

Completion of a PGY-1 residency or equivalent experience

Knowledge of chronic disease states and their management

Ability to work effectively within a team and engage with diverse patient populations
