Program Description:

Consistent with our philosophy that every child deserves a safe, loving, permanent family and the opportunity and preparation to thrive in school and in the world, Graham Windham appoints a team of professionals to support and advocate for children and youth who are on track to be returned home to their families, to be adopted or to be matched with an alternative permanent resource.

Principal Objective/Role Overview:

Youth Development Coordinators work with children and youth in grades 3K to 5 and are tasked with providing a range of supports to the Foster Care and Adoption Case Planning teams which focus on targeted areas of need including: applying for and securing housing for youth between 16-21 years old, and intervention on issues of problematic attendance and truancy, special education and, specifically, youth who have Individualized Education Plans (IEPs), family planning, and immigration for children and youth between grades 3K to 5.

Job Responsibilities:

• Educational Assessment & Planning:

• Complete education assessments and create success plans for children in foster care (3K–5th grade).

• Document all child centered related education outreach and advocacy efforts in the NYS Connections system of record.

• Maintain up-to-date educational case records and summaries for reporting as per NYS OCFS mandate through documentation of all case related education activities.

• Advocacy & Support:

• Ensure appropriate school placements and advocate for school stability under ESSA law.

• Assist with enrollment during transitions (Intake, program changes, hospital discharge, detention, RTC, etc.).

• Support students needing academic intervention, special education evaluations and services.

• Address school suspensions and safety concerns.

• Advocate for summer school for students at risk of retention.

• Collaboration:

• Work with case planners, clinical teams, foster parents, and caregivers using Solution-Based Casework to ensure children are achieving academic success.

• Attend all agency family planning meetings such as family team conferences, discharge meetings, and school-related meetings

• Support case coverage for all students in grades 3K-5 as need

• Monitoring & Documentation:

• Track enrollment, attendance, school performance transfers, transportation, and evaluations.

• Document advocacy efforts and progress in Connections (CNNX) and maintain education records.

• Student Enrichment:

• Connect students to programs for academic success, including tutoring for those behind.

• Lead middle school selection process for 5th graders. Assist with planning, attending all workshops

• Celebrate academic milestones and organize end-of-year school choice and annual ready-to-learn school events.

• Training & Engagement:

• Facilitate workshops for parents, staff, and foster parents on educational topics.

• Coordinate sessions like “The More You Know” to enhance educational knowledge

• Other education related activities as assigned

Qualifications

Position Qualifications:

• High school Diploma Required; bachelor’s degree preferred.

• Experience working with at-risk children, youth and adolescents.

• Computer literate

Salary & Compensation:

• Base Salary or Hourly Rate: $31.67/hour
