A Brief Overview
Evaluates, plans and administers therapy to patients who have speech-language and / or swallowing disorders in their homes.
What you will do
- The ability to comprehend spoken and written English, to communicate with clarity; work independently with non-combative and consistent behavior, good assessment and documentation skills, good technical procedure skills, the ability to interact with a mixed patient population, the ability to hear normal and abnormal speech sounds.
- Performs patient evaluations to determine the nature of the speech / language / swallowing disorder.
- Identifies patient's speech / language and / or dysphagia, develops plan of treatment with long and short-term goals to meet those needs and attain the patient's maximum level of functioning.
- Provides rehabilitative services for speech and language therapy to achieve identified goals.
- Observes, documents and reports to the physician / dentist and Home Care Case Manager the patient's reaction to treatment and pertinent changes in the patient's condition.
- Provides rehabilitative services for dysphagia and / or neurological or physical disabilities related to the patient's ability to swallow.
- Completes documentation on a timely basis and accurately documents history, home environment, physical findings. functional ability on initial assessment, follow up narratives and flow sheets, orders, case conferences, re-certifications, and discharges per policy.
- Communicates patient problems, needs, progress to Nurse Case Manager via verbal and written case conference note.
- Verbally instructs patient / family / caregiver and other health team personnel and family members in methods of assisting the patient to regain speech and language function and provide written exercises for follow up.
- Maintains visit frequency as ordered and notifies physician via verbal order of any changes in visit patterns or treatment plan.
- Communicates with managed care organizations for pre-authorization of services, documents communication, submits documentation as requested and follows authorized visit frequency.
- Participates in staff in-service meetings and other educational programs to keep abreast of current trends.
- Incorporates the physiological and developmental age of patients into the overall plan for the delivery of patient care.
- Completes weekly therapy statements and submits by Monday morning each week with documentation of visits.
- Maintains and respects confidentiality.
- Demonstrates compliance with the Hospital and Home care dress code policy by appearing well groomed, wearing ID badge while on duty and maintaining a professional appearance.
- Observes Hospital policy concerning Fire and Safety regulations, Infection Control Standards. and other rules governing conduct while at work.
- Demonstrates a commitment to own work schedule by adhering to attendance standards and obtaining coverage for cases during illness or vacations
- Complies with Home Care request for information required for annual contract renewal.
- Understands and participates in the Home Care Performance Improvement Plan and process.
- Provides care and services to reflect a commitment to the Holy Name Hospital Mission Statement.
- Performs additional related duties as the need arises.
Education Qualifications
Master's Degree Graduate of Masters program approved by the American Speech and Hearing Association RequiredCompletion of supervised Clinical Fellowship Year preferred. PreferredExperience Qualifications
Home Care experience PreferredKnowledge, Skills, and Abilities
Knowledge of testing and evaluation measures related to communication and swallowing disorders.Knowledge of Medicare, Medicaid and third party insurance reimbursement, State, Federal, JCAHO standards for home care.Job requires extensive standing, walking, stair climbing, writing, driving and phone use.Responsibilities include making home visits in various areas during inclement weather and exposure to variable environmental conditions in the home setting.Licenses and Certifications
ASHA - American Speech Language Hearing Association a Certificate of Clinical Competency in Speech Language Pathology. Pay Range : $39.50 / hour - $46.50 / hour
