As a Nurse Navigator, you will play a crucial role in collaborating with program managers and staff to achieve important health and wellness objectives. You will engage with various community sectors such as faith-based groups, educational institutions, and organizations to enhance the overall health of individuals, emphasizing a holistic approach that addresses body, mind, and spirit. Your responsibilities will include providing health education, facilitating screenings, advocating for health access, conducting support groups, managing cases, and reaching out to underserved populations. You will tailor your services to meet the distinct health needs of individual families, making a meaningful difference in their lives.

Required Qualifications :

• Bachelor's Degree in a health-related field, or 5 years of clinical experience in community health settings.
• Upon hire : California Registered Nurse License
• Upon hire : National Provider BLS - American Heart Association
• 3 years of healthcare experience.
• 1 year of clinical experience in community health.
• Experience in working with and managing volunteers.
