Your Responsibilities : Evaluate and assess patients' physical abilities to develop comprehensive, individualized treatment plans. Implement and monitor physical therapy interventions to ensure optimal patient outcomes. Collaborate with a multidisciplinary team to facilitate coordinated patient care. Document all patient interactions and treatment in the electronic medical record (EMR) system. Educate patients and their families about rehabilitation plans and home exercise programs. Requirements Master's or Doctorate degree from an accredited Physical Therapy program. Licensure or eligibility for licensure as a Physical Therapist in the state of practice. Strong interpersonal communication skills and the ability to work collaboratively in a team environment. Commitment to providing exceptional patient care and improving community health outcomes.
