Requirements :

• AZ DRIVERS LICENSE
• FAMILY ATMOSPHERE
• PROOF OF CAR INSURANCE (if wanting to run errands / doctor appts)
• FINGERPRINT CLEARANCE CARD
• CURRENT TB TEST RESULTS
• CPR CERTIFICATION (will accept on-line training)
• 1ST AID CERTIFICATION (will accept on-line training)
• 6 MONTHS EXPERIENCE PREFERRED
• 3 PROFESSIONAL REFERENCES
