Job Function:

• Provide office-based management for a geriatric patient population.

• Promote and implement geriatric care protocols and standards across all adult medicine providers within the network.

• Educate Internal Medicine residents on the principles of geriatric care, including through lectures and clinical teaching.

• Develop care transition models to support home health, family involvement, and post-acute care planning.

• Collaborate with Hospital Medicine and Emergency Department physicians to establish core competencies in managing the unique needs of geriatric patients.

• Create referral protocols to specialists commonly involved in the care of older adults, such as rheumatology, endocrinology, and cardiology.

Job Requirements:

Job Requirements:

• Board Eligible or Board Certified in Family Medicine or Internal Medicine, with completion of a Geriatric Fellowship.

• Active New York State medical license required.

• Current ACLS or BLS certification required.
