Responsibilities
The Supportive Care / Palliative Care Nurse Practitioner (SPCNP) collaborates with the HarmonyCares Medical Group to deliver comprehensive and quality palliative care services for patients under the direction of the supervising Regional Chief Medical Officer (CMO). Responsibilities include conducting patient visits for both new and established palliative care patients, coordinating their plan of care with designated staff, particularly the palliative care team. Visits may occur in patients' homes, facilities, and hospitals. The SPCNP will independently address healthcare concerns and goals of care, providing palliative services as needed while having access to collaborating providers for consultation.
