Seeking an experienced Primary Care Physician/Hospitalist to work with an adult medicine population in a high-risk clinic. The ideal Internal Medicine will have 2-3 years of experience. Great opportunity for a Hospitalist or PCP.

About the position:

• Care is for highest acuity patients; Some headed from hospital back to PCP; Some staying for months with team and many using the high-risk clinic instead of going to an emergency room for minor conditions
• Significant behavioral health overlay
• Will work closely with an advanced practice care team including nurse care managers, behavioral health specialists, and clinical pharmacists
• Decision-making is often team-based and very collaborative
• Case conferences about individual clients
• Must be an effective communicator in a fast-changing environment
• Willing to learn new practice management techniques in collaboration with population health medical director
• Highly flexible and a team player
• Able to communicate well in group settings as well as 1:1 with patients and family members
• Able to work collaboratively with other physicians and PCPs
• Will serve a diverse, multi-cultural, underserved patient population
• Base Salary plus incentive plan based on quality measures

• Call is 1:14 weekday and weekend
• Loan repayment through NHSC

• PTO is 240 hours per year
• One Week CME hours plus $2500
