Provides pharmacological information to the multidisciplinary health care team. Monitors patient drug therapies. Dispenses medications accurately and in a timely manner. Resolves questions regarding formulary and non-formulary medications, following policies and procedures. Provides dosing and therapeutic recommendations that take patient age / kinetics based on literature. Performs patient education, including information on proper dosing as prescribed. Ensures patients are aware of the importance of taking medications on time. Informs patients about potential side effects and interactions with other medications they may be taking, as per department requirements. Graduation from an accredited College of Pharmacy is required. A minimum of two years as a licensed pharmacist is required.
