Job Title : Registered Nurse I

Experience : Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience, depending on the specialty.

License : Must have license in hand.

Certifications : CPR, BLS

Must-Have :

• Proficiency in HomeCare HomeBase (HCHB) and Oasis, used within the last 6 months.
• Reliable transportation and current driver's license.
• Agency-required liability insurance.

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician orders.

Care is based on agency policies and procedures through the competent application of the nursing process.

Responsible to Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation within a 75-mile radius of assigned care center.

Mileage will be reimbursed at the IRS standard rate.

Must be comfortable with potentially long drives between patient appointments.

Types of cases include start of care, wound care, follow-up visits, and others.

All documentation must be completed within 24 hours of the appointment with the patient.

Delayed or incomplete documentation may impact billing and follow-up patient care.

Weekly productivity points expectation is 30 points, equating to approximately 40 hours of work.

Overtime must be pre-approved.

Must accept every patient on the schedule regardless of distance from previous appointment.

A health checklist is required.

Local candidates are accepted at the same rate.

Must be open to on-call rotations as needed.

No contract hours are guaranteed; work is subject to patient census.
