The Registered Nurse (RN) at Home Health conducts consultation and admission visits to evaluate patient's skilled care needs and coordinates their individualized care plan. This role involves administering skilled nursing care to homebound patients while adhering to physician's directives, performing assessments including OASIS, and ensuring compliance with healthcare standards. The RN, Start of Care (SOC RN) in Home Health performs consultation and admission visits to assess a patient’s skilled care needs and completes the admission assessment. Also completes resumption of care and recertification assessments with corresponding OASIS as required.  Skilled nursing care is administered to homebound patients at their place of residence under the direction of and in accordance with physician's orders.

• Able to establish effective, timely working relations with patients, families, staff and physicians
• Basic computer knowledge and abilities
• Must have demonstrable skills in the establishment of peripheral intravenous lines and administration of intravenous therapies
