Insight Global is seeking an experience Home Health RN to provide in-home care to patients in the Greater Charleston, SC area. This role combines clinical expertise with leadership responsibilities, providing high-quality care to patients in their homes while supporting and training caregivers. The ideal candidate is flexible, organized, self-directed, and passionate about improving patient outcomes.

The Registered Nurse will be required to travel to patients home around the Charleston, SC area- expect travel time to be up to 1 hour with possible increases as we expand our reach to patients in the future. When working in patients homes, RNs will be conducting patient assessments, creating care plan, creating task sheets, and training caregivers. Expect about 20 hours a week to be in patient homes. The RN will be expected to work onsite in the office outside of patient visits, the office is located in West Ashley.

Key Responsibilities

• Conduct comprehensive patient assessments and determine appropriateness for home health services.
• Develop individualized care plans in collaboration with case management staff.
• Create and maintain task sheets for caregivers to ensure accurate and consistent care delivery.
• Provide training and follow-up for caregivers to ensure compliance with care plans and documentation standards.
• Supervise and mentor additional RNs as the team expands (initially 1 RN within 6 months, with potential growth).
• Maintain direct communication with caregivers and clients to address concerns and ensure quality care.
• Spend approximately 20 hours per week in patient homes performing assessments, care planning, and caregiver training.
• Ensure proper documentation and adherence to regulatory requirements.
• Collaborate with office staff, including the Administrator and Scheduler, to coordinate patient services.
• Utilize Generations Homecare Software (experience preferred; training provided).
• Manage own schedule and travel efficiently.
