The clinician will provide 1 : 1 patient care and group counseling. The daily patient load will be approximately 50-60. The focus will be on serving adults aged 55 and older. The population served includes low-income individuals who are medically complex. Clinicians will work with patients who have mental health diagnoses, behaviors, and / or substance abuse issues. Candidates must have reliable transportation. There are no weekend requirements. First-time travelers are accepted. The role involves conducting biopsychosocial assessments and documentation for care planning. Candidates must have experience in case management and care coordination. The documentation must reflect an understanding of coverage criteria. The clinician will participate in care conferences and facilitate discharge planning. Communication with internal and external team members is essential. Candidates are expected to float as needed within the facility.
