Licensed Practical Nurse (LPN) Job Description
Location : A New Well, Inc. St. Charles, Missouri
Position Type : Full-Time / Part-Time
Reports To : RN Supervisor & Program Director
Position Summary
The Licensed Practical Nurse (LPN) provides high-quality nursing services to individuals served by A New Well, Inc., ensuring that all care is delivered in compliance with Missouri Department of Health & Senior Services (DHSS) regulations, Medicaid waiver requirements, and A New Well, Inc. policies. The LPN supports individuals in the adult day program, group home, and / or community-based settings by providing medication administration, health monitoring, documentation, and advocacy.
Essential Job Duties & Responsibilities
1. Clinical & Nursing Care
- Administer medications according to MAPA and Missouri DHSS standards.
- Complete daily health assessments, including vitals, skin checks, and symptom reviews.
- Provide wound care, first aid, and chronic condition support as directed by the RN.
- Monitor for changes in health status and take appropriate action immediately.
- Follow all physician orders, treatment plans, and nursing directives.
- Assist with ADLs (Activities of Daily Living) as needed to ensure dignity and safety.
- Ensure infection control procedures are followed at all times.
2. Documentation & Compliance
Complete nursing notes, progress notes, MARs, incident reports, and health logs in real time.Maintain accurate, timely, and compliant records per :Missouri DHSS
Missouri Medicaid Waiver RequirementsCARF standards (if applicable)A New Well policies & proceduresAssist with quarterly and annual health summaries.Track and document immunizations, physical exams, TB testing, and other medical updates.3. Medication Management
Maintain medication inventory, labeling, storage, and disposal.Conduct medication counts, reconciliation, and safe handling.Train direct support staff on medication protocols as allowed by Missouri guidelines.Communicate medication changes to staff, RN, and leadership.4. Communication & Coordination
Communicate any medical concerns to the RN Supervisor immediately.Serve as a liaison with families, guardians, physicians, pharmacies, and hospitals.Coordinate follow-up appointments and ensure completion of physician paperwork.Participate in team meetings, ISP meetings, and care-plan reviews.5. Health & Safety Oversight
Monitor for signs of abuse, neglect, or exploitation and report as required by Missouri law (mandated reporter).Ensure emergency plans are understood and followed.Participate in monthly and quarterly safety audits.Maintain a clean, safe, and organized nursing area.6. Training & Professional Conduct
Maintain an active Missouri LPN license in good standing.Complete required annual trainings (CPR / First Aid, Abuse / Neglect, Infection Control, HIPAA, etc.).Provide staff guidance on health-related issues.Uphold A New Wells mission, professionalism, and confidentiality standards.Qualifications
Current Missouri LPN license (required).One year of experience working with adults with disabilities, seniors, behavioral health, or long-term care (preferred).Experience in adult day programs, group homes, home health, or community-based services (preferred).CPR / First Aid Certification (or willing to obtain).Ability to use electronic documentation systems.Strong communication, teamwork, and problem-solving skills.Physical Requirements
Ability to lift up to 2540 lbs.Ability to bend, stand, walk, push / pull, and assist with transfers when needed.Ability to respond quickly in emergencies.Work Schedule
