California Licensed Admissions Coordinator

A company is looking for a Remote Admissions Coordinator Long-Term Care & Rehabilitation (RN or LVN / LPN).

Key Responsibilities

Coordinate and oversee admissions and transitions of care for members entering long-term care or rehabilitation settings

Review clinical documentation to support appropriate placement based on member needs and Medi-Cal criteria

Serve as the primary liaison between health plans, SNF admission teams, and hospital discharge planners to ensure timely placements

Qualifications

Active, unrestricted California RN or LVN / LPN license (required)

Minimum of 2-3 years of experience in home health, long-term care, or intake / placement coordination

Working knowledge of California Medi-Cal and long-term care guidelines

Proficiency with EMRs and care coordination platforms

Ability to work independently in a fully remote environment
