This is a full-time opportunity working weekdays from 8 AM to 5 PM, Monday to Friday, with limited after-hours on-call rotation. You will care for patients in Kalamazoo, Calhoun, and / or St. Joseph counties.

Key Responsibilities of the Hospice RN / Care Manager :

• Serve as the primary point of contact for patients and families, guiding them through each step of their hospice journey.
• Deliver and document skilled, hands-on nursing care tailored to each patient's individualized Plan of Care, whether at home, in assisted living, or another facility.
• Conduct initial and ongoing assessments to track patient conditions and adjust care as necessary.
• Collaborate with a multidisciplinary team, including physicians, LPNs, CNAs, social workers, chaplains, and others, to create and refine personalized care plans.
• Provide emotional, physical, and spiritual support to both patients and their loved ones, offering education, comfort, and bereavement guidance.
• Educate caregivers on how to safely and confidently care for their loved ones, providing both practical training and emotional reassurance.
• Communicate significant updates regarding patient conditions to the care team, ensuring timely and appropriate interventions.
• Maintain accurate, current clinical documentation and adhere to infection control and compliance standards.
• Support the organization’s quality improvement efforts and scheduling initiatives to ensure care remains compassionate and efficient.
• Assist patients and families in understanding and navigating essential topics such as medication administration, hospice philosophy and services, symptom and pain management, and the end-of-life process.
