Position : Clinical Pharmacist I

Location : Hawthorne, NY 10532

Schedule : Monday–Friday, 9 : 00 AM–5 : 30 PM

Work Arrangement : Onsite

About the Role

This role supports daily pharmacy operations while the team transitions to SM Cloud. You’ll join a collaborative environment of pharmacists, technicians, and shippers to ensure timely and accurate order fulfillment. The position focuses on clinical decision-making, dispensing oversight, patient counseling, and ensuring compliance with all regulatory and quality standards.

Key Responsibilities

• Review and assess physician prescription orders for accuracy, dosage, interactions, route, and stability.
• Oversee medication filling, compounding, receiving, and dispensing.
• Ensure complete patient documentation, including allergies, medical history, labs, and medication lists.
• Coordinate with nurses and physicians on clinical decisions and lab follow-ups.
• Deliver patient counseling, education, and clinical monitoring.
• Perform clinical interventions and monitor patient response to therapy.
• Support technical and clinical inquiries from healthcare teams.
• Document all clinical and communication updates in patient charts.
• Maintain compliance with state / federal regulations and Joint Commission requirements.
• Assist with training pharmacy technicians, interns, and staff.
• Participate in quality improvement, in-service programs, and educational activities.
• Support the Pharmacy Manager with new procedures and operational needs.
• Participate in on-call rotation.

Required Qualifications

Preferred : Additional state pharmacist licenses.
