Family Nurse Practitioner, Outpatient Community Health (FQHC)

(Only QUALIFIED Healthcare Professionals accepted) Family-NP - Longstanding community Health Center is looking for a Family Nurse Practitioner with 5+ years of experience in outpatient primary care to join an amazing group of providers in a neighborhood-based clinic.

This position cares for a solid patient panel including many pediatric patients. The Family Nurse Practitioner provides general medical care & treatment to center patient's bye performing physical examinations & preventative health measures. Order, interpret and evaluate diagnostic tests to identify and assess patients' clinical problems. Provide medications for chronic illnesses and adjust meds as needed.

The care team includes Family Physicians, Internists, Pediatricians, OB-GYN Physicians, Nurse Practitioners, and Dentists. Center also provides integrated behavioral health, nurse care management, optometry, podiatry and asthma|allergy services.
