Job Overview

Location / Schedule : The position is collocated between Leland House and Pearl House, residential treatment facilities, located in Oregon City / Gladstone, OR. The schedule for this position is Monday through Friday, day shift, 7 : 00 a.m. to 3 : 30 p.m.

Program Description : Cascadia operates several licensed residential treatment homes for adults experiencing mental health challenges. Our homes provide psychiatric treatment, personal care, skills training, and case management. Employees have an opportunity to provide services in a single staffed location or a location with multiple staff scheduled simultaneously. The goal is to prepare residents to live in a less restrictive environment. Referrals are accepted from counties and the state, typically for individuals leaving the state hospital. We operate sites in Multnomah County (N, NE, SE Portland) and Clackamas County (Oregon City, Gladstone).

Position Description : The Medication Services Coordinator is responsible for medication room related services at the assigned facility. This includes providing day to day provision of medication administration, PCP care coordination services, as well as quality assurance and oversight of the medication services performed on site. This position involves program procedure development and treatment plan implementation of an established population of individuals with severe and persistent mental illness.

Essential Responsibilities :

This position description is not intended to be an all-inclusive list of responsibilities, skills, or working conditions associated with the position. Management reserves the right to modify, add or remove duties as necessary.

  • Manage the administration, documentation, and evaluation of the medication management program. Train, coach, and consult with all Residential Counselors who dispense medication.
  • Oversee medication administration room, including inventory, supply orders, tools, and materials.
  • Conduct periodic audits of medication room services to ensure quality of service, and documentation compliance with all applicable internal and external regulations. Identify and recommend program improvements as necessary.
  • Maintain timely and effective coordination and communication with site medical staff (LMP and nurse); schedule resident appointments, follow through on medication orders, coordinate client consultations, and disseminate key treatment issues.
  • Provide leadership and primary oversight for MAR completion, 'Cycle Fill' documentation / ordering, pharmacy coordination and communication, as well as narcotic medication handling procedures.
  • Schedule medical services appointments, with primary care providers, specialists, dental services, and hospital / E / D's.
  • Coordinate with LMP, and clinical leadership for effective provision of self-administered medication education and skills building curriculum for all residents, including provision of direct training to residents.
  • Facilitate program goals and development, while meeting productivity and program expectations, and operating within the program budget.
  • Collaborate with program leadership and applicable community partners to develop and implement the regulations, policies, and procedures under which the program operates.
  • Maintain knowledge of, ensure compliance with, and review all applicable federal, state, county, and city regulations and approved internal policies and procedures.
  • Document all services in a professional manner using the agency EMR system.
  • Carry partial or total responsibility for developing and implementing assigned operational functions within the medication room.
  • Provide positive role modeling and mentoring to peers while fostering a client centered approach by developing, maintaining, and advancing the consistent use of clinical best practices.
  • Participate in program development and trainings.
  • Provide input on programmatic documentation / procedural requirements.
  • Provide oversight and continuity of care, as well as communication and task assignment during all staff / team meetings, shift changes, and related procedures. Coordinate with oncoming shift regarding resident concerns, building / program needs, and any issues relevant to ensure the smooth operation of the program.
  • Provide input toward the therapeutic treatment plan in collaboration with the case manager.
  • Complete annual employee training requirements on a timely basis as indicated in the licensing requirements for the facility as well as agency requirements.
  • Perform other duties as assigned.

Qualifications

Education :

  • Bachelor's degree in a behavioral science field from an accredited college or university or three years of full time professional experience working with adults with severe and persistent mental illness. Demonstrate the competencies to become credentialed as a Qualified Mental Health Associate (QMHA), as outlined in OAR 309-016-0005.
  • Experience :

  • Experience working with adults with severe and persistent mental illness preferred.
  • Specialized Knowledge, Skills and Abilities :

  • Understanding of components of maintaining a Trauma Informed Care environment
  • Operate computer hardware and software at a level needed to effectively perform job functions
  • Ability to enter and retrieve data
  • Ability to prepare simple reports
  • Other :

  • Possess a valid driver's license, three year driving history; clean driving record, ability to pass Driver Motor Vehicle (DMV) check and complete the Cascadia online Defensive Driving course.
  • Ability to perform CPR, First Aid, and properly use AED equipment. CPR / First Aid certifications are required and must remain current.
  • Working Conditions

    Work will be performed in a secure (locked) residential setting. The nature of the work dictates that the employee is alert and attentive during his or her assigned shift. Rest breaks and meal periods are subject to interruption. Periodically, staff may be required to work a double shift if the need arises, due to the staffing requirements within each program. There is moderate to high risk of personal injury / illness due to violent behavior, exposure to disease, biohazards, noise and contaminants.

    Mental Demands : The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach.

    Physical Demands : Activities and structure support community integration assist program residents in learning socially acceptable behaviors.

    Must be approved to provide transportation utilizing agency vehicles so that residents may access the community. The Medication Services Coordinator must also have the ability to assist residents in participating in a variety of active leisure and recreational events, as well as skill building outings and accessing community resources. The Medication Services Coordinator must have the ability to assist the team in providing safety to all residents and staff in potentially volatile situations.
