Medical Care Coordinator (Remote)

We're seeking a Medical Care Coordinator to support members as they navigate their healthcare needs, benefits, and available resources. This role is ideal for someone who is patient-focused, organized, and passionate about improving access to care.

What You'll Do

• Serve as the first point of contact for members in care management
• Help members understand benefits, coverage, and care options
• Conduct proactive outreach to address gaps in care and prior authorization needs
• Provide disease-specific education (adult & pediatric) under a licensed Care Manager
• Identify social determinants of health and help remove barriers
• Manage a caseload of 150–300 members with stable but complex or rising-risk conditions
