[job_card.job_description]Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents Obtain medical history, current medications, and symptoms Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare / insurance guidelines Coordinate the start of services (nursing, aides, equipment, medications.) Coverage area : Travel throughout the territory is required - Covers Edina, Monticello, Forest Lake and Woodbury branches
Schedule : 4-10 hr shifts w / on call rotation
