Primary Care Physician - PACE Program

Schedule : Full-Time | Rotating On-Call (Remote)

Language : Spanish proficiency highly preferred

What You'll Do

As a Primary Care Physician within the PACE model, you'll provide comprehensive medical care tailored to the unique needs of older adults-helping them stay healthy, active, and in their homes as long as possible.

Key Responsibilities :

• Act as the primary care provider for an assigned panel of PACE participants
• Deliver direct medical care across multiple settings : PACE center, participants' homes, and skilled nursing facilities
• Initiate, manage, and monitor medications, treatments, and labs
• Collaborate with the Interdisciplinary Team (IDT) to develop individualized care plans
• Provide ongoing support during hospital admissions, discharges, and transitions of care
• Educate participants and caregivers on chronic conditions, preventive care, and overall wellness
• Refer participants to specialty providers when needed
• Perform routine in-office procedures when clinically appropriate
• Maintain thorough, accurate documentation in the Electronic Medical Record (EMR)
• Participate in value-based care initiatives , quality improvement, and team development
• Contribute to broader organizational growth, innovation, and patient-centered outcomes
