Pay Rate / Benefits : The pay range for this role is generally $41,600 - $46,600 annually , commensurate with relevant experience and qualifications and in alignment with internal equity. Community Health Action of Staten Island (CHASI ) drives dramatic improvements in the health of New Yorkers by feeding people who are hungry, healing families broken apart by violence, and bridging the gaps between people and the compassionate health care they deserve. CHASI provides outreach, education, prevention, and direct support services for populations most affected by health disparities – people with low or no income, low-income people with chronic illnesses, people with criminal justice involvement, people who use drugs, domestic violence survivors, people of color, and the LGBTQ community.
POSITION SUMMARY :
The Hotspotting Opioid Program is an evidence-based program in which a care management model will be led by certified recovery peer advocates and clinical professionals with the objective of providing person-centered services to those at risk of opioid overdose in a safe and collaborative way. The program’s focus is to reduce opioid harm, promote a continuum of recovery, and address critical social determinant of health factors.
Under the supervision of the Director of Harm Reduction, the individual will be responsible for providing client outreach, interventions for clients, provide crisis consultations, maintain client case records, linkages to care, referral follow up. Case Managers will document interventions according to all funder and agency requirements, submit statistical data and reports, attend weekly supervision, participate and facilitate case conferences, conduct staff trainings on substance abuse topics, and perform other additional tasks as delegated by supervisor.
