Responsibilities :

Conduct comprehensive assessments of client needs.

Develop individualized, goal-oriented care plans.

Coordinate services with healthcare providers and community resources and collaborate with interdisciplinary teams.

Monitor care plans, adjusting as needed.

Provide support, education, and advocacy for clients and families.

Maintain accurate documentation and respond to client needs and emergencies.

Qualifications : RN

3+ years in elder care, care management, or healthcare.

Strong organizational, communication, and problem-solving skills.

Empathetic advocate for older adults.

Valid driver’s license and reliable transportation.
