Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The MDS Assessor will play an integral role in helping us to achieve our vision : to consistently exceed expectations and set the standard for excellence in residential and community care.

Responsibilities

• Lead and oversee all aspects of the MDS process, ensuring compliance, accuracy, and optimal reimbursement.
• Monitor CMI, lead related meetings, and regularly communicate Quality Measures to the interdisciplinary team.
• Review MDS assessments to ensure accurate coding, conduct PDPM reviews upon admission and prior to submission.
• Collaborate with rehab to verify correct primary diagnoses.
• Review UB04s, manage ADR responses and appeals, coordinate with Livanta, and submit documentation to the billing company.
• Participate in audit responses, run Medicare Part B reports, and actively participate in key facility meetings including Medical Board, Therapy, QAPI, and Utilization Review.
• Ensure and maximize proper reimbursement while maintaining compliance with federal and state guidelines.
