Cobble Hill Health Center is a not-for-profit long- and short-term care facility providing services to the disabled and elderly for over 40 years. The MDS Assessor will play an integral role in helping us to achieve our vision : to consistently exceed expectations and set the standard for excellence in residential and community care.
Responsibilities
- Lead and oversee all aspects of the MDS process, ensuring compliance, accuracy, and optimal reimbursement.
- Monitor CMI, lead related meetings, and regularly communicate Quality Measures to the interdisciplinary team.
- Review MDS assessments to ensure accurate coding, conduct PDPM reviews upon admission and prior to submission.
- Collaborate with rehab to verify correct primary diagnoses.
- Review UB04s, manage ADR responses and appeals, coordinate with Livanta, and submit documentation to the billing company.
- Participate in audit responses, run Medicare Part B reports, and actively participate in key facility meetings including Medical Board, Therapy, QAPI, and Utilization Review.
- Ensure and maximize proper reimbursement while maintaining compliance with federal and state guidelines.
Qualifications
Current New York State RN license.Three or more years of experience in a long-term care facility as an MDS Coordinator.Knowledge of Medicare and Medicaid criteria and assessment regulations.Excellent listening and communication skills.Ability to complete documentation in a timely manner, with attention to detail and accuracy.RAC Certification and prior leadership experience are strongly preferred.What We Offer
