Schedule : Full-Time / Day Shift. 9am-6 : 30pm with every 6th to 7th weekend coverage.

Location : Covering Admissions in Western Suburbs, including Hinsdale, LaGrange, Darien, Willowbrook, Western Springs, Bolingbrook, Addison, Naperville, Bloomingdale, Streamwood, Lemont, and Orland Park.

The Role You'll Contribute

The Admissions Nurse is responsible for responding to and visiting with patients who have been referred for hospice consultation and or admission to AdventHealth Hospice Care. The Admissions Nurse assesses the appropriateness / qualifications of the patient for Hospice services and informs the patients, caregivers, and family members of the hospice services available with the organization.
